What to Expect when I arrive to Synergy?
- Nurse will greet you when you arrive
- Head-to-toe assessment by your nurse
- A visit with your physician
- Meet your care coordinator
- Comprehensive evaluations by the treatment team
- Meet your therapists who will each assess your abilities and set goals
- Meet with a Clinicsal dietician
- Meet with a wound care specialist (if needed)
Your Team Will Include
- Physical therapist: Based on your needs, your PT will work with you on building your strength, increasing your endurance, increasing your range of motion and in enhancing your mobility
- Occupational therapist: Your OT will also help you use specialized equipment as needed and will help you become more independent in everyday living.
- Nurse: Your rehabilitation nurse will work closely with you and your therapists and will help you practice what you learn in your therapies.
- Psychologist: You will be seen regularly by a psychologist who will determine your individual treatment and ongoing care needs and collaborate with your rehabilitation team
- Speech therapist: Your speech and language therapist will help you regain your ability to communicate Therapeutic recreation specialist (if needed) Your therapeutic recreation specialist will work with you on building confidence and social interaction skills, learning to plan and do activities independently or with minimal assistance
- Dietician: Your dietician will assess your diet, work with the dietary staff to ensure you receive proper nutrition
